• Sansio Rewards Home Health Customers who use Technology to Reduce Avoidable Hospitalizations


    Sansio, a leading provider of SaaS-based healthcare solutions, today announced a challenge to its HomeSolutions.NET home care software customers. To mark the introduction of new real-time HomeSolutions Hospitalization Risk Management tools, Sansio will reward its home care customers who reduce their Acute Care Hospitalization Rate.

    Sansio will work closely with its customers to utilize the new HomeSolutions Hospitalization Risk Management features which provide a data-driven approach to:

    • Identify patient risk for hospitalization based on predictive modeling
    • Enable electronic alerting and workflow to manage these patients, and
    • Report on hospitalizations in real-time with dashboards and drill-down capability

    Real-time Acute Care Hospitalization performance will be measured in HomeSolutions on a quarterly basis. Customers who show improvement from the prior quarter will be recognized as a Hospitalization Reduction Partner and receive materials from Sansio which they can use to help publicize their improvement.  These customers will also be entered into quarterly drawings.

    Hospitalization Risk Alert

    The HomeSolutions Hospitalization Risk Management tools use an algorithm to automatically calculate a patient’s Hospitalization Risk Score and identify their Hospitalization Risk Level of Low, Medium, and High in real-time, at any point after intake. This drives electronic workflow in HomeSolutions and enables the agency to apply strategies proven to reduce avoidable hospitalizations such as patient/caregiver education, front loading visits with preventive interventions, medications management, telehealth, and fall reduction programs.

    Several statistical views and dashboards accompany the Hospitalization Risk Management functionality and customers can drill down into the patient’s EMR in the HomeSolutions Virtual Chart to audit hospitalization events and determine if anything could have been done to avoid them.  The Hospitalization Risk Management tools were released to all customers at no additional cost to HomeSolutions customers as part of Sansio’s Software as a Service model that continually brings additional value to customers.

    Home Health and Avoidable Hospitalizations

    Hospitals began realizing 1% hospital readmission penalties on October 1, 2012, and as reported by Kaiser Health, over 2,000 hospitals will forfeit about $280 million in Medicare due to high readmissions for patients with Congestive Heart Failure, Acute Myocardial Infarction, and Pneumonia. According to Medicare data, the United States has an 18 percent rate of hospital readmissions within 30 days of discharge and as many as 76 percent of these are preventable. It is estimated that as much as $25 billion could be saved each year if these unnecessary readmissions were avoided.

With forthcoming changes to how ACH rates are publicized on Home Health Compare, it is important for our customers to be able to identify improvement trends using real-time, consistent data so they can better manage to their hospitalization reduction strategies.”

    Public reporting on Home Health Compare of ACH Rates are slated to change from OASIS to CASPER per the 2013 PPS Proposed Rule issued by CMS.
